The Peer Support Specialist/Coach provides a variety of recovery-oriented to individuals who have self-identified as desiring peer support, in order to promote greater indepedence, community intregration and recovery from mental illness as an expanded function of Elmhurst Home, Inc. The Coach includes a wide range of support which provides advocacy that contribute to a client's ability to engage in ongoing treatment. Successful candidate assist in the development of peer-led drop in programming to expand Elmhurst Home, Inc. program. 

Responsibilities

  • Work  with peers to identify wellnesd and recovery goals. Assist peers in creating Wellness plans that identifies that individual's self-determined goals. 
  • Exemplifies recovery principles in order to show by example that recovery is possible and attainable. 
  • Work with peers on specific objectives identified on the Wellness plan, ensuring that these specific objectives always refer back to the individual's ISSP for billing purposes. 
  • Attend and positively participate in required team, agency and interdiscipilinary meetings. 
  • Coordinate and consults with other team members to keep the team informed of issues and progress. 
  • Ensure timely completion of all required paperwork.
  • Work to increase the level of culture sensitivity, awareness and competency regarding Person-Centered and Peer Delivered Services. 
  • Document all peer contact at the time of service
  • Educate and advocate for the values and principles of the Recovery Model. 
  • Work with Team Lead to develop and support new programming
  • Recieve regular weekly supervision form the Team Lead, seeking additional supervision whenever necessary. Mininum requirement two hours of documented supervision per month. 
  • Participate in interdisciplinary team meetings and group supervision as required. 

Min. Requirements  - Education & Experience

  • Documented successful completio and /or development plan of the Certified Peer Recovery Mentor - Michigan (CPRM-M).
  • Work experience in the health human services or behavioral sciences field, or current enrollment toward a degree in these field.
  • One year of substance abuse treatment and/or mental health disorder experience. 
  • Strong organizational skills. 
  • In-depth knowledge of community resources such as housing, medical, and other needed support services specifically related to the population served. 
  • Group facilitation experience a plus.
  • Famility with the 12-step model and subustance abuse.
